Home-based care includes several distinct services with different payers, licences and workforces. Naming them precisely is the first step in any analysis.
Medicare home health
Skilled nursing, therapy, social work and aide services for homebound patients under a physician plan of care, delivered by certified agencies. Intermittent, episode-based and clinically focused.
Personal care and home care
Non-medical help with bathing, dressing, meals and household tasks, paid by Medicaid waivers, long-term care insurance or families. Delivered by aides through licensed or registered agencies depending on the state.
Hospital-at-home and home-based primary care
Acute-level care delivered at home with daily visits and remote monitoring under a Medicare waiver, and physician or nurse practitioner house-call practices for frail patients. Both are growing rapidly.
Worked Example
A client says it wants to ‘get into home care’. The consultant lays out the four service types, their payers and licence requirements, and the client realises its clinical strength fits Medicare home health while its market opportunity is in private-pay personal care. Two different businesses, two different plans.
